Application for Provisional Permit for Completed Stockwater Pit or Reservoir
May be used for a completed stockwater pit or reservoir when: (1) the only
purpose is stockwater (2) the storage capacity is less than 15 acre-feet
with an annual appropriation of less than 30 acre-feet per year, (3) the
reservoir is located on a nonperennial flowing stream, and (4) and it is
constructed on and accessible to a parcel of land 40 acres or larger and
owned or under the control of the applicant.

Used for all changes in ownership of entire water right (including change
of entire water right in connection with a real estate transfer). Do not
use this form if you are a user of a municipal or irrigation district water
system.
Form No. 641, for a water right that will be divided.
Form No. 642, for a water right that will be exempted (reserved) from a sale of land.
Form No. 643, for a water right that will be severed from land. A severed water right does not involve a land sale.
